The attached (Inler is a routine action Senting a utility location permit to Verizon Commuvicatiore Im

pedestals, posts, markers, signs and othe fixturesin, along, under and across the surface of Stillwater
Avenue and Gilman Road. Maine Public Utilities Commission regulations require Council approval of
such installations m public ways.
We have dealt with Verizon on severed similar installations with full satisfactory results. Approval is
recommended. Copies of me permit application and layout sketch are attached.

TMT the attached utility location permit is herby granted to Verimn Communications,
for Ne propose of installing and maintaining buried cables, conduits, manholes, handholes, wdN the
and cables therein, pedestals, posts, markers, signs and other fixtures in, along, under and across
the surface of Stillwater Avenue and Gilman Road, provided that all work is done in accordance with
applicable City codes and ordinances and any additional requirements of the City Engineer.
NOTE: Following are the public streets refined to:
Stillwater Avenue, beginning at the intersection of Gilman Road, continuing nonhmsterly
-748 feet of buried conduit
-1 Manhole
-1-114 foot lateral to existing marshals on Bangor Mall Blvd.
Stillwater Avenue, beginning at the intersection of Gilman Road, continuing southwesterly
-2868 feet of buried conduit
-2 Manholes
-1— 58 foot lateral into the Bangor Mall.
Gilman Ronal, beginning at the intersection of Stillwater Avenue, continuing northwesterly
-1028 fast of buried conduit
-1 Manhole
To be placed at a minimum depth of 24 inches between the shoulder bennslcurbs and a minimum depth
of 12 inches elsewhere.
In addition, placement and maintenance of underground Islands, conduit, manholes, handholes, cables
and wires in, along, under and across the above or intersecting public way for the purpose of making
connections with such poles and buildings as necessary for distributing purpose.

pedestals, posts, markers, signs and other fixtures in, along under and across the surface of the
following public streets and highways:

THIS APPLICATION IS BEING SUBMITTED WITH PUBLIC NOTICE BY PUBLICATION
Said cables/conduits shall be placed substantially in accordance with the plan Bled
herewith marked Vetizon New England Inc. No. 210657 dated April 25, 2001 and shall be
placed at a minimum depth of 24 inches between the shoulder berms/curbs and a minimum depth
of 12 inches elsewhere.
Also for permission to place and maintain underground laterals, conduits, manholes,
handholes, cables and wires in, along, under and across the above or intersecting public ways for
the purpose of making connections with such poles and buildings as it may desire for distributing
purposes.

ORDERED: That permission be and is hereby granted VERMON NEW ENGLAND
INCORPORATED (fk.a. NEW ENGLAND TELEPHONE AND TELEGRAPH COMPANY)
to place and maintain buried cables, conduits, manholes, bandholes, with the wires and cables
therein, pedestals, posts, markers, signs and other fixtures in, along, under and across the surface
of the following public streets and highways as requested in application of said Company dated
the 25a day of April, 2001,
Stillwater Avenue, beginning at the intersection of Gilman Road,
continuing northeasterly - 749 feet of buried conduit

Also that permission be and is hereby grunted the VEBTZON NEW ENGLAND INC. to
place and maintain underground laterals, manholes, handholes, cables, conduits, and wines in,
along, under and across the above or intersecting public ways for the purpose of making
connections with such poles and buildings as it may desire for distributing purposes.
The foregoing permission is subject to the following conditions'.
1. The laterals, conduits, manholes, handholes, pedestals, ports, markers and signs shall
be of such material and construction and all work done in such a manner as to be
satisfactory to such municipal officers as may be appointed to the supervision of the
work, and a plan showing the location of buried cable laid and/or conduits constructed
shall be filed with the City when the work is completed.
2. Said Company shall indemnify and save the City harmless against all damages, costs
and expense whatsoever to which the City may be subjected in consequence of the
acts or neglect of said Company, its agents or servants, or in any, manner arising from
the rights and privileges granted it by the City.

3. In addition, said Company shall before a public way is disturbed for the placing ofits
buried cable and/or conduits, extort¢ its bond in a penal sum of Five Thousand Dollars
($5,000.00) (reference being had to the band already on file with said City
conditioned for the faithful performance of said agreement and of its duties under
this permit).
4. Said Company shall comply with the requirements of existing by-laws/ordinances and
such as may hereafter be adopted governing the construction and maintenance of
buried cables, manholes, conduits, poles, wires, pedestals, posts, markers and signs so
far as the same are not inconsistent with the laws of the State.
TRIS PERMIT IS BEING GRANTED WITH PUBLIC NOTICE BY PUBLICATION
It is further adjudged and voted that satisfactory proof of validity of the applicants notice
or procedures required by statute has been given upon the application of said Company to place
buried cables, conduits, manholes, with the wires and cables therein, pedestals, posts, markers
and signs in, along under and across the public streets and highways of Bangor, Maine,
designated by this permit.
