SPECIFIC ACTIONS AROUND THIS INTIATIVE: A. Provide J public clinics per month for the purpose ofebildhood immunization. Thiscan be done in conjunction with Adult Immunizations. C. Document aggregate numbers of Childhood Vaccine Administration on the provided spreadsheet with accurate data collected and provided to the Maine Immunization Program at least quarterly. Monthly reporting is encouraged. 2. The Maine Immuebation Program desires to provide certain Adult High Risk groups with access to free Hepatitis A & B Vaccine. Provide immunization to identified high risk adulm, in cooperation with the Bangor City Public Health Department STD clinic, who meets Ne above mentioned criteria with recommended doses of Hepatitis A and/or Hepatitis B vaccine. The vaccine for these high risk groups is being provided free of charge for the purpose of this agreement through the Maine Immunization Program. B. Strive for completion of Soo ifthe entire Hepatitis vaccination Series in these identified High Risk Adults.
