Department applied for and was successful in obtaining gran[ funds from Maine Fire Training and
Education to fund the cast of the replacement panels.
This Resolve will accept and appropriate the funds, which total $8,537.50. Council previously provided
authority to apply for the grant and to establish the necessary account

WHEREAS, the Fire Department's training facility bum room is in need of replacement
burn panels; and
WHEREAS, Maine Fire Training and Education has granted $8,537.50 to the City of
Bangor to fund the replacement of the burn panels a[ the Training Facility;
and
WHEREAS, this funding will be used to replace the bum panels.
NOW, THEREFORE, BE IT RESOLVED THAT: THE CITY COUNCIL OF THE QTY OF
BANGOR THAT the $8,537.50 in funding from Maine Fire Training and Education for the
purpose of replacing the bum panels for the Fire Department is hereby accepted and
appropriated to the aobunt established for this purpose.

Dear Chief Cammack:
I am pleased to inform you that your community has been selected to receive a 2002
Firefghter Training Facilities Maintenance/Safety Grant in the amount of $8,537.50.
Enclosed are the following:
a. 2 original Grant Agreements (Signed by me)
b. 1 Maintenance Project Monitoring Form
C. 1 Report of Expenditures Fops
Read the grant agreement carefully, and then do the following as soon as possible, but
by 11/15/02 at the latest '
1. Sign both Grant Agreements on the "Project DirectoP line. Keep one for your
flies.
2. Complete columns one and lard of the Project Monitoring Fenn, listing what you
will need to accomplish as your project progresses (Column 1), and the dates
you expect your project to be done. The completion. date on the .project
monforing fops should be- the same as the date listed on your grant application.
Sign the form at the bottom. Keep a copy. You'll need it to trackprogress of
your project.
3. Fill in Column 1 (State Funds Budget) and Column 4 (Local Funds Budget) of the
Report of Expenditures, listing the breakdown of anticipated expenditures. Keep
a copy.
4. Mail one signed Grant Agreement, the signed Project Monitoring From, and the
Report of Expenditures to MFTBE In the aroused envelope.
As soon as MFT&E receives your signed Grant Agreement, Project Monitoring form and
Report of Expenditures form, they will advise the SMTC Finance Director to forward
your maintenancelsafety grant funds. You should receive those funds within 30 days
after MFT&E staff receives your paperwork.' '

provisions may result in revocation of grant funds. Please call'MFT&E Training

SAFETY ISSUES GRANT PROGRAM
GRANT AGREEMENT
Southern Maine Technical College through its Maine The Training and Education Department,
(the"Deparlwou7 hereby awards a grant to the city of Bangor (the "City') in the amount of. '-
$8,537.50:
In accepting the grant, the City agrees to the. following coditimu: -
1. The City will owaibate funds, property and services trnding$8,537.50, as described in
the project application, for atoml projected cost of $17,075.00
2. The Grein together with local funds shall be used to execute the project as proposed in
the City's giant application, (the'Project`%which is incorporated by mutates into this
Grant Agreement.
3. The expendnve ofgrant funds will be ncompliance with the described Project.
4. Any proposed variance from the project as described in the grant application ofmore
than 10 percent in itemized categories or anvariance fromthe Project as described shall
votmkeplace unless approved in advance by the Departments. -
5. The expenditures ofgrant funds by the City will be in accordancew it its customary
financial procedures and the requ'veawnts of Maine law.
6. Up -to -dare fmmciat and other records relating to the project will be mentioned by the
City and made available to Department staff for review.
7. The City will designate a Project Manager who will have supervisory responsibility for
the project and will function as the haisdn person with the DeparlmeN.
8. The City will establish dates for completion of identified phases offie project on the
Connotation Monitoring form that is attached and node a part of this Grant Agreement.
9. The project Director shall complete the Construction Monitoring form as indicated on the
form, indicating the reasons for arty delay nary phase of the project
10. Tbe.Project Dvarorwill wpplythe following documents to the Department's staff
transfer (a) during planned or spontaneous evaluation visits:
a) Up-to-date Maintenance Project Monitoring From (colanum 3M)
h) Uppto date dish rs®ent records (City's customary financial records).
c) Documentation of my deviations from the approved project budget...
. The project Director will accompany the Department's staffmember(s) during evaluation visits.
11. Financial records of the Project will be included in the annual audit of City expenditures
and any deviations or discrepancies reported to the Department, with a copy of the
auditor's report.
12. The project will he. completed by the project m npletion date in the application.
13. Purchase of materials and nonemployee services shall be done either through
competitive bidding or negotiations, according to eatablishd City procedure. No
officer or employee shall prafil individually from the Project.
14. The project shall be completed in accordance with National Fire Protection
Association (NFPA) standard 1403,'' ide to Building Fire Training Centers", 1997

15. The Department may withhold or recover payment of or pmt of the grant if the City
is found, after an adjudicatory proceeding or adjudication to be violation of the Maine
Human Rights Act (5 M.RS.A.§ 4551 e[ sea.) or the Federal Civil Rights Act (42
U.S.C.§ 1991 et sm.) during the Project period.
Ib. The Project Director shall submit to the Depermerd a completed Report of
Expenditures, the form atolad ad castle a part of this Gram Agreement, within 30
days of completion of the Project.
17. Payment of the grant shall take place within 30 days of the return of signed Grant
Agreemart to the Depaitment
This (hard Agreement is approved by tla City of Bangor in consideration of Training Facility
Maintenance Grant Award by Southern Maine Technical College.
