Responsible Department: Health and Welfare
Commentary: The City's Deparbnent of Health and Welfare currently receives an annual grant of
$10,000 from the state to assist with the immunization program. The State has proposed to expand the
program by adding a public health immunizadon educator with a goal to increase immunization hopes in
Me five northern counties of Maine. Strategies Include vtsiting primary are and pediatric offices,
attendance at health fairs and other public forums. Maine's immunization rates have dropped from
fifieenM to twenty-seventh nation wide aver the past several years.
The State proposes to increase the grant amount by approximately $42,500 for bre last quarter a
calendar year 2005 and to fund Me grant at $100,000 for calendar year 2006. The 2005 funding
increase would allow for hiring the educator, establishing an office and acquiring necessary office
equipment There is no cost or matching funds required of the City.
The Government Operations Committee has reviewed and recommends approval of this grant
amendment.

BE IT ORDERED BY THE CRY COUNCIL OF THE CITY OF BANGOR, THAT
the City Manager is hereby authorized to execute an amendment to the current
Immunization Grant Agreement between the Oty of Bangor and Maine Department of
Health and Human Services for the period of time through December 31, 2005, at an
estimated amount of $42,500, and in a form approved by the City Solidtor or designee.

The Department of Health and Human Services bas approached us to expand our
Immmiaatum program by adding a public health immunization educator. They propose
to amend our current $10,000 unreal January—Dccemba 31, 2005 contract by adding a
$40-50K increase. The 06 centrad would be increased from the normal $10,000 gmt to
$100,000.
From now until January in, we would be expected to do the following:
• Participate in 2-3 state ping meetings. Pordmd is being askedW
participate as well.
• Submit a grant e#ensionfor 2006 that would include new and expanded
deliverables.
• pocume office space(we have room with Glona Cote's retirement) and
equipment (PC, Phone, etc.)
• Post positions and advertise
The goal with this position is m increase immunization rotes (Maine has gone from 5'" in
the US to 27'" in the past few years). The strategies would be to visit primary care and
pediatric offices, have a presence at health fans and other public forums, etc. The
northern 5 comties of the state would be the territories involved.

• It would contribute much needed infiushucume to our lmhrunizatim
Department (office he1p, contribution to administrative overhead, etc.).
• The cost to adminiswr$1W,000 Want is not much more than a$10,000
Grant.
• We have been given the OK to frontload contract by doing things like
paying out fm'06 with'05 contract extension, buying needed supplies
end equipment with'05 money, leaving more fox PmOncel in'W
• We feel confident we can accomplish what would be expected. Outputs
would be how we are measured initially with outcomes measured down
thenad.
The state worts to work with existing commedurs(Pbrtlaed and Hang ) initially, but may
look to add a few more sites in the fame. This would likely mean our territory woold
shrink m future yeers. Fording comes from CDC and appears relatively stable.
The State is seeking a response to their proposal. We would recommend that the City
respond affirmatively. If the City and State are in agreement, a formal gent amendment

and acceptance and appropriation of these funds would need to be appmvM by the
Council.l will plan to attend the meeting to answer any questions that you may bave.
