This is the first year that Mr. Kosciusko has acted in this capacity. The term of this contract Is from March 1, 2009 through February 28, 2005. The amount of compensation is $20,000. Of this $5,000 is from the City of Bangor, $5,000 is from the Department of Behavioral an Developmental Services and $10,000 has been contributed by local care providers. The following tasks /activities support the ongoing Continuum of Care process throughout the year as designated. As a separate component, assisting the CDC with development of the annual HUD SuperNOFA application is required as outlined. The CDC Coordinator will also need to identify and apply to funding sources to sustain his/her staff position on an ongoing basis. ONGOING: Resource development: Monitor HUD, HHS, SAMHSA web9[es for relevant announcements/information related to Continuum of Care resource needs. Distribute announcement of funding opportunfties through the CDC. If applicable, solicit applications and present to the COC (Section 7D 1. Project solicitation, Project Committee) Update changes in CDC membership roster: Individuals and organizations (utilizing Section 2D: Organizations Involved, Membership Committee) Reach out to organizations in Perwbscot County as directed by Membership Committee (Section 21): Organizations Imiohred, Membership Committee) Orient new members to the mission and operations of the CDC. Assist them in selecting committee membership. (Section 2D: Organizations Involved, Membership Committee) Update and maintain an updated list of the actions taken by the COC to: 1) end chronic homelessness and 2) end other homelessness (Section 3A.1. and Section 3 B.1. CDC Goals and System Under Development). Keep updated, maintain and distribute the list of prevention, outreach and supportive services including services in place, planned and description of access to services, through regular contact with member organizations and other service providers In Penobscot County. (Section 3E. Fundamental Components: Service Activity Chart, HMIS Committee) 06-91 Update, maintain and distribute the current inventory of all emergency shelter, transitional and permanent supportive housing including target population and bed capacity. (Section 3E. Fundamental Components: Housing Activity Chart, HMIS Commldbee) Using the HMIS and coordinating with the two other Maine COC's, update and maintain a current database of housing beds and of the homeless population and subpopulatlons (Section 4 Housing Gaps Analysis and Homeless Population and Subpopulabons Charts, HMIS Committee) Under the direction of the Project Committee, assist with implementation of a system for COC to evaluate the performance of and ongoing need for projects seeking renewal. Project Renewals, Project Committee) Review and update the COC strategy for increasing and monitoring participation in mainshneam programs: including eligibility, enrollment and ensuring reception of services. Prepare the agenda, facilitate the meeting, record, prepare and distribute minutes of the meeting. Coordinate development of the grant preparation timeline with the grant writer. Maintain at least dally contact with the gram writer for coordinating and supplying Information needed. (Section BA Project Leveraging, Project Committee) Calculate the level of participation In the COC process for each member organization meeting attendance, committee participation (Section 2 D: Organizations Involved, Membership Committee) Review and update the list of specific accomplishments of the COC over the past 12 months in Implementing its planning and coordinating strategy (Section 1: Accomplishments) Summarize the actions taken by the COC over the past 12 months to: I) end chronic homelessness and 2) end other homelessness (Section 3A.1. and Section 3 B.1. COC Goals and System Under Development). Assist in the review and preparation of goals and action steps for the next 18 months. (Section 3A.3 and Section 3 8.2. COC Goals and System Under Development). Update and prepare the annual point In time gaps analysis chart. (Section 4 Housing Gaps Analysis and Homeless Population and Subpopuladons Charts, HMIS Committee) Keep current an updated inventory of data sources, reports, etc. to be utilized each year. Using the APR's, calculate the level of participation in mainstream programs and employment (Section 8C Mainstream Resources: Participation, Mainstream Resources Committee) Update the Mainstream Resources chart following the guidelines in the SuperNOFA. (Section 8D Mainstream Resources, Mainstream Resources Committee) Under the direction of the Project Committee, assist agencies in the preparation of applications for the SHP, S+C and SRO Exhibits. (Project Committee) 04-91 AINING ONGOING STAFF POSITION FUND Identify public, private, federal, state, tool funding sources to supplement and sustain funding for this staff posltbn. Develop and complete written proposals for funding this staff position. Ono awarded, assist with monitoring and satisfying compliance requirements of the various funding sources. SUPERVLSION and PERFORMANCE EVALUATION: Reports directly to the Dire=r of Health and Welfare. Performance shall be evaluated annually by the Staffing Committee of the GPOOC using the performance evaluation Instrument adopted by the City of Bangor. REQUIREMENTS Internet access and E-mail.
