Commentary: The purpose of this Order is to authorise the City Manager to enter Into a
Personal Services Agreement with Sally Healey for the purpose of coordinating the HUD
Homeless Continuum of Care Project. Ms. Healey previously held this position and has
agreed to return.
The term of this contract is from November 14, 2005 through July 31, 2006. The amount of
compensation is Twenty Thousand Dollars ($20,000.00) plus up to an additional Five
Thousand Dollars ($5,000.00) for required training, technical assistance and travel. The City
will be contributing $10,000 and other participating agencies/organizations will contribute
Me remaining balance of up to $15,000.

PROFESSIONAL SERVICES AGREEMENT
Homeless Continuum of Care Community Coordinator and Grant Writer
his AGREEMFNf is tnadethis qday of November 2005, by and
between the CITY OF BANGOR, a municipal corporation and governmental entity
located in Bangor, Maine (hereinafter "the City") and SALLY HEALEY, of Bangor,
Maine(hereinafter "Healey"). The parties hereby agree as follows:
1. Healey shall serve as the Homeless Continuum of Care (COC) Community
Coordinator and Gm rt Writer from November 1, 2005 through November 1, 2006
as an independent contractor. She shall report to the Bangor Director of Health
and Welfare.
2. Healey's responsibilities shall include:
A. Coordinating all activities related to the annual HUD Homeless
Continuum of Care Grant application for the Greater Penobscot
Continuum of Care (GPCOC).
B. Organizing, convening, facilitating and documenting grant application
planning meetings of local homeless service providers, including, but not
limited to, the Greater Penobscot Continuum of Care stoning committee;
C. Organizing and conducting a "point in time" survey in cooperation with
other Maine COC's to include, bat not be limited to; consumers, service
providers, emergency response personnel, businesses, the faith
community, government officials and other interested pares; aM
preparing an analysis of me data for the annual COC application;
E. Collecting and collating documentation of the past year's activities and
providing this information m appropriate GPCOC committees for analysis
and action.
F. Research additional funding opportunities appropriate for the
implementation of the goals and objectives of the GPCOC, including
continued loading of the position of the Homeless Continuum of Care
Community Coordinator and Gant Writer. -
G. Perforating other related duties as assigned, including but not limited to
the attached single -page Scope of Work GPCOC Part-time Homeless

3. The City shall pay Healey a want of twenty thousand dollars ($20,000.00) payable
in four installments of $5,000.00 each. The first installment shall be paid the
week of Dec 31, 2005, the second imammem shall be paid the week of March 3l,
2006, the third installment shall be paid the week of 3me 30, 2006, wed the fuel
installment shall N paid the week of October 31, 2006.
4. This Agreement may beterminated by the City upon the breach of Healy of any
of the terms and conditions contained herein.

The specific tasks will vary according to the requirements delineated in the annual Super
NOFA add to the supports and services colonel by the working committees of the
GPCOC. In genial, areas of responsibility will include (but are not limited to):
r Prepare,including Romancing according to JND requirements, all sections of
Exhibit One with input add assistance from GPCOC committee chairs and
members.
Provide staff support, as needed, for GFCOC committees. Assist comminees in
implementing, managing and updating GPCOC goals and objectives.
• Attend GPCOC plenmy meetings; assist Chair with agenda preparation and
distribution; keep attendance data; prepare and distribute minmes.
♦ Provide orientation for new members; maintain the GPCOC list serve and
membership roster.
