'6oL66�i�aa RB)ERA SPECIFICATIONS OF SERVICES TO BE PROVD)ED Funds are provide] for the Bangor Immunization Program W provide from walk-in immunization clinics. All clinics will follow the Standards of Immunization Practices. Clinics will he open W all persons without regard to age, sea, race, religion, sexual orientation, residence, income, or disability. Clients will be screened for vaccine need and contraindications per Standards. After screening, all vaccines will be administered simultaneously. Clients will receive a State of Maine Immunization record and clinic records will be kept on fine at the Health Department. No child will be refused Stare vaccine i£they me unable to pay the administration Re. No physical exam will be required to receive vaccine and no physical exams will be offered on site. Clients without primary physicians will be referred to local physicians. Standing orders will be cleaned and reviewed on a yearly basis. Emergency equipment will be available at all clinics and will include: ammonia in emborm, Epinephrine (adult and pediatric) oxygen and airway supplies. The Bangor Immunization Program will be available to see patients two afternoons and two mings per week by appointment. Scheduled appointments arc for those clients who are unable to allend a scheduled clinic. Clients will be seen and screened using the same Standard for the walk-in immunization clinic. The Bangor Immunization nurse will be available on a technical resource basis W the Maine Immunization Program and the nurse will respond to request for information of technical nature in regards to immunization practices by the Some Immunization Program. Indicator and Measure GOAL STATEMENT -The Bangor Immunization Program will provide immunization service to children, adolescents, and addle to assist the State of Maine Immunization Program in meeting the year 3001 immunizationgams. MDICATOR-Provide throe walk-in immunization clinics and clinic will be available W see patients two summers and two trimmings per week by appointment and all clinics will follow the Standards of Immunization practices. MEASURE -Will be measured through quarterly reporting of number of consumers, clinic locations and dates, and immunization one reports. Service Soec'6cation The Bangor lmmonimtion Program will provide servImS to low socioeconomic populations, and the statewide yellow never clinics are two additional activities pedcnned by the Maine lmmun nation Program. These activities are designed to supplement services provided by the private sector throughout Maine.
