HomeMy WebLinkAbout2011-07-11 11-234 ORDERItem No, Dace: July 11, 2011 Item/Subject: Order, Authorizing the City Manager, on Behalf of the Health and Community Services Department, to Apply for Two Community Transformation Grants in Conjunction with the State of Maine Center for Disease Contras Responsible Department: Health and Community Services The State of Maine Center for Disease Control is submitting two applications for Federal Community Transformation Grants and has asked the City of Bangor to partner with them for both applications. The first gram is designed to address chronic diseases and would serve the Penquis Region. The second grant is designed to address the disparate health needs for the 12 towns that make up the Healthy Maine Partnership. Both grants deal with directly with public health issues in the Bangor Region and will assist in addressing and reducing a variety of chronic diseases. Additionally, under the grants, Bangor will also act as lead fiscal agent for the larger of the two grants. These grants do not require a match. ORDERED, That the City Manager, on behalf of the Health and Community Services Department, is hereby authorized to apply for two Community Transformation Grants in Conjunction with the State of Maine Center for Disease Control, one in the amount of $150,000 for five years and one in the amount of $75,000 for five years. m cin COORCIL' July H. 2011 F"S8 r..n IT 239 To: Government Operations Committee From: Shawn Yardley, Director of Health and Community Services Subject: Authority to Apply for Grant Funds with State of Maine CDC and to Act as Lead Fiscal Agent Date: June 24, 2011 The State of Maine Center for Disease Control (CDC) is submitting an application for a federal Community Trairsf000ation grant focused on public health. Since at least 50% of the grant must go to work at the community level, the Maine CDC is proposing to work with the existing public health districts to receive the grant funds and oversee the grant activities within the districts. To do this, they need the help of one experienced and trusted partner in each public health district to serve as the Lead Fiscal Agent for the - -- podion of grant funds allocated for work in the district. The goal of the new funding is to reduce the rates of chronic disease by achieving progress in • healthy weight proper nutrition physical activity • tobacco use • emotional well-being use of high -impact prevention services The City of Bangor has acted as fiscal agent on a number of districtwidegrants over the past several years and is viewed as a trusted and effective collaborator. If the State is successful in their application, $150000 a year for 5 years would come into our District to serve the people of Bangor and throughout Penobscot and Piscataquls. If approved, a Council Order will be prepared for the July ltd City Council meeting 1.] 2'V To: Government Operations Committee From : Shawn Yardley, Director of Health and Community Services Subject: Authority to Apply for Grant Funds in conjunction with State of Maine CDC Data: June 24, 2011 The State of Maine Center for Disease Control ( CDC) is submitting an application for a federal Community Transformation Grant (CTG), a portion of which seeks to increase. access of consumers to community prevention programs to support healthy choices and contribute to wellness. As part of the community level work supporting this strategy, priority is given to high risk subpopulations aswellas persons with health disparities_ - The primary objective is to. increase the uptake of community resources by the consumer to improve their lifestyle choices and prevention self manage their chronic disease set management. This is achieved by 1. Developing a comprehensive referral protocol and integrating it into practice: 2. Developing patient navigation reprisal and training.. and 3. Increasing wnsumeraocess to communityresources The City of Bangor has been providing this level of service within the Parcels district addressing chronic disease prevention; this grant would extend this service further to improve the health of specific subpopulations within our region. It the State is successful in their application. $75,000 a year for 5 years would be available in our district to serve the people of Bangor and throughout Penobscot and Plural counties. Staff recommends that authority be given to apply for these grant funds in conjunction with the State CDC. If approved, a Council Order will be prepared and included on the Julyllth City Council agenda.
