Responsible Department: Health & Community Services Commentary: This order authorizes the City Manager to apply for a $185,000 grant from the AstraZeneca Healthcare foundation to provide funding for work with the Eastern Maine Healthcare Systems, Penobscot Community Health Community Care Teams, and other Beacon sites to reduce rates of cardiovascular disease in our region through education and on-site services. The approach to reducing cardiovascular disease will largely emphasize physical activity, nutrition, and reducing tobacco dependence, with some systems level change and direct service being provided in each area. The physical activity and nutrition component will look at people in congregate housing; people living independently or homebound, but relying on community meals; and people living independently and able to self prepare meals. The tobacco component will focus on providing comprehensive technical assistance in developing tobacco free cultures within community organizations. This was reviewed and unanimously recommended for approval by the Government Operations Committee on March 13, 2012. By Me Oty Counts/ of the O)ty of Bangor: ORDERED, THAT the City Manager is hereby authorized to apply for a $185,000 AstraZeneca Healthcare Foundation Grant to work with the Eastern Maine Healthcare Systems Community Care Team, PCHC Community Care Team and other Bangor Beacon Community sites to implement system, policy, and environmental changes to improve physical activity and nutrition, reduce tobacco use and reduce cardiovascular disease in our region. The grant period is one year from December 2012 to December 2013.
