Item/Subject: ORDER, Authorizing the City Manager to Execute a Memorandum of
Understanding with Eastern Maine Healthcare Systems for the Partnerships in Community
Health Grant Application.

Commentary: This Order will authorize the City Manager to enter into a Memorandum of
Understanding on behalf with Eastern Maine Healthcare Systems, the applicant organization for
a multi -county Healthy Maine Partnership submission to the US CDCs Partnership in Community
Health grant program. The Bangor Public Health and Community Services Healthy Maine
Partnership is one of the applicant partners.
The CDCs Partnerships in Community Health Grants are three year awards being made to
communities to decrease tobacco use, increase access to healthy food and beverages, increase
physical activity, and increase chronic disease management supports.
Further action will be pursued if the grant application is successful, and when an award is made
to Eastern Maine Healthcare Systems. This was reviewed and recommended for approval at the
July 7, 2014 Government Operations Committee meeting.

BY THE CITY COUNCIL OF THE CITY OF BANGOR:
BE IT ORDERED, that the City Manager is hereby authorized to enter into a
memorandum of understanding with Eastern Maine Healthcare Systems, the Applicant
Organization for a Multi -County Healthy Maine Partnership Grant Application to the US Centers
for Disease Control, a copy of which is attached.

Memorandum of Understanding
Northern Maine Rural Collaborative for Healthy Communities
(NMRC)
Purpose: EMHS is the lead applicant, on behalf of Healthy Sebasticook Valley Coalition
(HealthySV), the Bangor Region Community Health Coalition, and for the "Northern Maine Rural
Collaborative for Healthy Communities (NMRC)" partners, in response to CDC-RFA-DP14-1417
Partnerships to Improve Community Health initiative. This Memorandum of Understanding
describes the nature of commitment between the collaborative partners and the resources that
they will bring to the project.
WHEREAS: EMHS is a regional health care system committed to improving the health status of
all Maine people; and is the parent legal entity for the Healthy SV Coalition, a program of
EMHS/ Sebasticook Valley Health, and a founding partner of the Bangor Region Community
Health Coalition, first established in 1993.
WHEREAS: The below named collaborative partners have an interest in coordinating efforts
across a seven (7) county rural region of Maine to implement strategic activities to address risk
factors that undermine community health;
This agreement establishes a framework for collaboration between EMHS and eleven (11)
Healthy Maine Partnerships and other partners across seven counties to launch the Northern
Maine Rural Collaborative for Healthy Communities project (NMRC).
Goals
Utilizing evidence -based practices and multi -sectoral community engagement, NMRC will launch
a three year project to:
1. Reduce tobacco use and exposure
2. Improve access to nutrition
3. Increase opportunities for physical activity
4. Establish linkages to chronic disease prevention and risk reduction opportunities
Expectations / Deliverables of EMHS:
EMHS will work with Key Personnel/Representatives of each partnering collaborative to:
Align efforts to implement SMART objectives and key strategies to reach priority
populations in each of the local communities within the local collaborative jurisdiction;
Assess community resources that can be utilized in the project;
Provide meeting support, project planning and the establishment of local objectives and
measurement opportunities.

EMHS will provide:
Contract Administration and Fiscal management of the CDC Cooperative Agreement, and
negotiate sub award contracts with individual collaborative partners for direct program
costs, and third party Program Evaluation services;
Program Management for implementation of selected strategies to meet Annual
Objectives by individual collaborative partners across the seven county region;
Communication Plan implementation to effectively support efforts of partners and share
success stories. EMHS will utilize resources as recommended by CDC, track and report
activities annually, and evaluate communication strategies;
Expectations / Deliverables of collaborative partners:
The Collaborative partners named below will take responsibility to:
Be the lead mobilizing entity in the local community, engaging local champions and
organizations in the effort;
Maintain a steering committee to guide the local effort, Implement local goals and
objectives with the support of multi -sector partners and the steering committee,
Establish an implementation plan over the term of the project,
Coordinate local media support,
Plan and implement local promotional events,
Develop a reward and recognition program for individuals or organizations who have
provided exemplary support for the initiative;
Provide periodic reports to EMHS related to local activities emphasizing success and
lessons learned.
Match:
Partners receiving sub -award funding will be expected to contribute matching funds averaging
25% of the award funding received.
Outcomes:
NMRC will contract with a third party Evaluator to measure success in achieving short-term
outcomes as outlined in the Work Plan.
Term
The Northern Maine Rural Collaborative for Healthy Communities project is ready to begin upon
Notification of Award by the Centers for Disease Control, anticipated on or about September 30,
2014 and will officially launch all planned activities upon signing of the Cooperative Agreement
between CDC and EMHS. The project period will be for three years, consistent with the date of
the Cooperative Agreement.

Authorization
The signing of this MOU is not a formal undertaking; it implies that the signatories
share common missions to improve health in their communities through evidence -
based strategies to: a) increase access to healthy food, physical activity, b) increase
access to resources to reduce chronic diseases, and c) decrease the use of tobacco;
and demonstrates intent to partner with EMHS/Bangor Region Health Coalition to
implement the NMRC program.
Upon Notice of Award and signing of a Cooperative Agreement with CDC, EMHS will
negotiate formal agreements in the form of subawards with collaborative partners.
