Item/Subject: Order, Authorizing the City Manager and Fire Chief to Execute an Ambulance
Billing Service Agreement with the Town of Baileyville and the Baileyville
Ambulance Corps

Commentary:
The Town of Baileyville and the Baileyville Ambulance Corps have requested that the Bangor Fire
Department provide ambulance billing services for them. This Agreement is similar to those the
City has with seven other communities and service providers. This work can be accomplished
with existing staff and will generate approximately $1,500/year in additional revenue.
At its April 14th meeting, the Government Operations Committee reviewed this request and
recommended approval to the City Council.

(TITLE.) Order, Authorizing the City Manager and Fire Chief to Execute an Ambulance
Billing Service Agreement with the Town of Baileyville and the Baileyville
Ambulance Corps
BE IT ORDERED BY THE CITY COUNCIL OF THE CITY OF BANGOR, THAT
the City Manager and Fire Chief are hereby authorized, on behalf of the City of Bangor,
to execute an Ambulance Billing Service Agreement with the Town of Baileyville and the
Baileyville Ambulance Corps under the terms and conditions of the attached agreement.

Re: Billing Agreement for Baileyville Ambulance Corps
Enclosed is a copy of a billing agreement between the Bangor City Fire
Department and BaileyvilIe Ambulance Corps. They are requesting for us to
enter into an agreement with them to provide ambulance billing services. We
currently have 7 similar agreements with other services and communities.
This would generate approximately $1,500.00 in revenue by providing this -
service to Baileyville Ambulance Corps. We are asking for your approval to,
enter into this agreement with the Baileyville Ambulance Corps.

AGREEMENT
THIS AGREEMENT executed in duplicate this day of
2009, by and between the BANGOR CITY FIRE DEPARTMENT of Bangor, Maine and
BAILEYVILLE AM13uLANCE CORPS. of Baileyville, Maine.
WHEREAS, Haileyville Ambulance Corps. provides ambulance service to its
customers; and
WHEREAS, the Bangor City Fire Department has the capability of billing for
ambulance service; and
WHEREAS, The Town of Baileyville desires that Bangor City Fire Department bill
Baileyville Ambulance Corps. customers for ambulance services on behalf of The Town of
Baileyville.;
NOW. THEREFORE, it is agreed as follows;
This agreement shall be for a terns of six months. It maybe terminated by either
party for any reason upon thirty days written notice. At the expiration of the term, this
Agreement may be extended by agreement of the parties for an additional term of six
months under the same terms and conditions as set forth herein.
The Bangor City Fire Department shall bill each customer of Baileyville
Ambulance Corps. three times. Bangor City Fire Department shall be responsible for the
cost of forms, envelopes and postage.
RESPONSIBILITY OF BAILEYVII,LE MANCE CORPS
Baileyville Ambulance Corps. shall purchase billing software from Sweet Soft for
the use of Bangor City Fire Department during the term of this Agreement.
Baileyville Ambulance Corps. shall be responsible for providing to Bangor City
Fire Dent correct information regarding its customers. The Town of Baileyville and
the Baileyville Volunteer Ambulance Corps. shall keep and hold harmless Bangor City
Fire Department from and against any and all claims, demands, suits, judgments, costs and
expenses asserted by any person or persons by reason of Baileyville Ambulance Corps.' s
actions hereunder.
Baileyville AmbUlance Carps. shall be responsible for responding to customer
inquiries.
PAYMENT
Baileyville Ambulance Corps. shall pay to Bangor City Fire Department the sum of
fifteen dollars ($15.40) for every ambulance run sheet submitted to Bangor City Fire
Department for billing. _
