Item/Subject: Order, Ratifying City Staff Action Submitting a Letter of Intent to Apply for and
Accept Funding in the amount of $20,000 from the Maine Health Access
Foundation

Commentary:
This order ratifies a letter of intent to apply for and accept grant funding in the amount of
$20,000 from the Maine Health Access Foundation. The City of Bangor Health and Community
Services Department would be the lead applicant to apply for Phase One of Maine Health
Access Foundation Achieving Better Health in Communities. Healthy Community Grants
Program. This project would bring together a diverse group representing public and private
organizations across multiple industries and sectors to work collectively to identify 1-2 critical
health issues that warrant attention in the Penquis region. The partners include Eastern Maine
Healthcare Systems; Eastern Maine Development Corporation; Penquis; Penobscot Community
Health Care; St. Joseph Healthcare; Community Health & Counseling Services; Bangor Area
Homeless Shelter; Town of Orono; Eastern Area Agency on Aging; Wellspring; Charlotte White
Center; Greater Bangor Chamber of Commerce; Bangor School Department; University of
Maine; and Pro -Elder Consulting, LLC. The proposed service area would be the Healthy Maine
Partnership service area including Bangor, Brewer, Clifton, Eddington, Glenburn, Hampden,
Hermon, Holden, Levant, Newburgh, Orono, Orrington, and Veazie. The amount of funding
would be $20,000 and the term of the grant would be November 1, 2013 — October 31, 2014.
The Letter of Intent needed to be submitted by April 12, 2013, so this Order would ratify action
taken by staff to meet the deadline.
