Contingent Account 61-01-07 to Education Account 71-01-00.
STATEMENT OF FACT
This request is for the purpose of nakinq available to permanent hourly
employees Blue Cross -Blue Shield coverage far dependents. The School Cowdttee
has voted to authorize this coverageby August 1st if possible.
