By the city cone! of W City Of Beaver: ORDERED, THAT the City Manager be and hereby is authorized to accept the attached Proposal from Metcalf & Eddy, Inc. for the design of Modifications to the bar screens at the Eenduskeag Pump Station, and be it further ORDERED, that the sum of $4, HOO.O be transferred from the Reserve for Future Construction Account - Sewers to cover the coat of said study. STATEMENT OF FACT A very hazardous condition exists on the maintenance of the bar screens at the Eenduskeag Pump Station, wherein a worker may lose his life or be seriously injured in the event of a broken drive chain. This condition has been cited by an Inspector for the State Department of Labor, and must be corrected by designing some sort of protection for the workers. Frawley City Engineer City Hall Bangor, Maine 04401 Dear Mr. Frawley: In response to your request, we are pleased to submit our proposal for engineering services in connection with the bar screen operation and maintenance at the Kenduskeag pump station and the treatment plant. Scone Of Services We propose to provide the following services: I. Review the bar screen shop drawings and the as - built drawings. 2. interview the plant personnel concerning the maintenance and repair problems. 3. investigate .the existing condition and operation Of the bar screens, 4. Revlew.the bar screen service conditions and the maintenance and repair problems with themanu- facturer. 5. Prepare a cast estimate for temporary and long- term recommendations to remedy the problems at bath installations. 6. Prepare a letter report summarizing the Engineer's conclusions and recommendations. ReSLonsibilities Of the Cit It would be the responsibility of the City to accomplish the following at no cost to us: NEW YORX RALOBVO CNI"GO Mr. Sohn T. Frawley - Tune 21, 1978 1. Provide all available information pertaining to this project. 2. Provide access and make all provisions for us to enter upon public and private land as required to complete this project. - 3. Designate in writing a person to act as your .representative with respect to this work. Time for Completion The time require9 to perform the services provided under this - proposal would be approximately two months from the date of authorization to -proceed. Compensation Our fee for the services provided under this proposal would be on a time -charge plus expense basis, payable monthly as charges accrue and would be the sum of the following: Salary cost times a multiplier of 2.25. Nonsalary expenses times a multiplier of 1.15. Salary cost is defined as salary and wages paid to personnel for time chargeable to the project plus a percentage covering -- allowances for sick leave, vacation and holiday pay; taxes and insurance premiums based upon payroll; and medical and retirement benefits. Nonsalary expenses include such typical expenses as cost of: transportation and subsistence; long-distance telephone and telegraph; printing and reproduction; computer time and programming coats; Identifiable supplies; outside consultants charges; sub- contracts for services such as surveys, subsurface investigations, and testing by commercial laboratories; and charges by review- ing authorities. It is suggested that you budget $4,800 for our services under this proposal. This figure would not be exceeded without your prior approval. In the event payment is delayed beyond 60 days from the date of invoice, interest would be charged at the prime rate plus one Mr. John T.
