THAT the Director of Health and Welfare is hereby authorized
and directed, on behalf of the City of Bangor, to execute an
agreement, a copy of which is on file in the office of the City
Clerk, with the State of Maine Department of Health A Welfare,
providing Forthe. continued operation of the Penobscot-Piscataquis
WIC Program as described therein for the period October 1,-1984 -
September 30, 1985; and be it further
ORDERED,
THAT the Director of Health and Welfare is hereby authorized
and directed to submit requests for payment pursuant to the terms
of said agreement on behalf of the City of Bangor.

Penobscot-Piscataquis WIC Program
Rider A- Fiscal Year 1964
A. The Bangor City Health and Welfare Department 'WIC Program
distributes instruments (drafts) to clients on a monthly basis. Via
encourage clients to pick up their drafts on or about the same day
each month. This practice is also in effect at our eleven outreach
sites. To reinforce this requirement we write the date of the next
scheduled appointment on the inside of all WIC ID folders. As
required by federal regulations, we issue 15 day notices to clients
failing to participate. Mailing drafts to clients is done only
:lien extenuating circumstances occur. Additionally, we maintain
-a-daily logofall outgoing mail., is.affixedto-all-mail
at the Treasurer's Office, City Hall.
A waiting list is available for persons applying to WIC
when funds are not available to serve the total needs. This agency
has adopted a screening process (pre -application papers), which
may be filled out in this office or mailed to perspective clients.

D. The Director of public Health and Welfare for the City of
Bangor is the Administrator of the WIC Program. -
The Program Director is responsible for the daily operations of all
aspects of the program.
The Nutrition Consultant is employed to oversee all education and
nutrition information dispursed by the nutrition staff. The
Consultant. is also responsible for certification procedures performed
by the Nutrition Aides.
The Nutrition Aides ape_responsiblefor the certification of the
clients, as well as oerforming secondary and high risk contacts.
The Office Manager oversees clerical duties.
The Intake Clerk is responsible for most clerical, communications,
and public relations of the program.
The Check Clerks issue drafts as well as performing secondary
contacts on each uIC client on a monthly basis. They are also re-
sponsible for vendor monitoring, and Galan Payment Systems business.
The 'Rork -Study Students will be responsible for group nutrition
education. and some clerical activities.
M. The Penobscot-Piscataquis WIC Program will provide nutrition
educationservicesto all participants. Daring the certification
process each recipient will be designated as in need of a high risk
or secondary contact -by the Nutrition Aide.
Secondary contacts All be made on a monthly basis at the.
tine of check issuance. A demonstration, hand-out, and brief
discussion of a particular subject selected by the Nutrition Con-
sultant tall be provided to all recipients; (parents, guardians,
or caretakers) both.at the main office and at outreach sites. All
nutrition education services will be recorded on a specified sheet
placed in the front of each "older, aswell as oil the Golan turnaround.

High risk counseling will be made as often as deemed
necessary by the Nutrition Aide. Additional counseling may be
provided by other health service agencies by referral-. Consistency
in nutritional information will be ensured by philosophies agreed
upon by area health service providers.
F. During the 1985 contract year the only outreach efforts
made will be through referrals, as we are pretty close to maximum
caseload.- Agencies with whom we plan to continue close contact
are Bangor Public Health Nurses, State Public Health Nurses, Family
Practice, ib24C Prenatal Clinic, Family Planning,- State/local immun-
ization program, EPSDT, Southern Penobscot County Preschool Project,
Department of Human Services, area obstetricians and pediatricians.
G. One of our check clerks is responsible for all -vendor -mon-
itoring activities. Vendor contract renewals are staggered, there-
fore she is scheduled to visit a certain section of our two -county
area. each month to monitor and renew contracts. The Director
usually handles any client/vendor complaints.
H. As mentioned above, this program enjoys the unique privilege
of .mrking with or towards goals established by state/local Public
Health Nurses, Eastern Maine Medical Center prenatal clinics, EPSDT
programs of Penquis CAP, Inc., Rural Pediatrics Program of WIC,
state/local immunization programs, and various health-related
educational programs. - -
