THAT John W. Flynn, City Manager, is hereby authorized
and directed, on behalf of the City of Bangor, to execute a
Grant Agreement, a copy of which is on file in the Office of
the City. Clerk, with the State of Maine Department of Human
Services providing anew grant of funds for purposes of
providing maternal and child health services in the City
of Bangor.
Statement of Fact: The purpose of this Order is to authorize
the acceptance of a grant from the State of Maine Department -A
of Human Services to fund one public health nurse position,
and to pay related costs for the City's Maternal and Child -
Health Services program. _

ARTICLES OF AGREEMENT
The State of Maine, Department of Human Services, hereinafter materials to as the "DopanmenGand Elie

wircements hereinafter mentioned, the Department and Ihn Banken agree to amydy with all terms :tad can

The following are the terms of the Agrnemenb -
1. THEGRANT
This Grant consists of the Ankles of Bass nt, Rider A—Specification of Work to be Performed, and
Hitler B—The Budget Section. This grant contains the entire agreement of the parties and neither Dairy shall
be homed by any statement or represadminm not tratairvxl herein except as provided for in Article 14.
The parties agree to comply with all applicable Federal statues and regulations, and with applicable
State Deficits.
2. AUTHORIZATION
The Grantee represents that it possesses legal aulhnrity to en Mr into a Grant agreement with [leDepart
men; that a resolution, motion ON similar action has been duly adontnl or passed as an official act of Elm
Grantees governing body, augu riEing execution of the Grant settlement and directing and Enthralling the
person identified as the official representative of the Grantee to actin connectlnn with the Grant and to per
vide such additional information as may be required.
3. TERM OF GRANT
This Grant is effective commencing July 1, 1984 _ _ antl ending

4. GRANTAMOUNT
Grant Award of 21,480.00 is cooditionial on the availability of State and Federal
Hands.
During the period of this Agreement, the Grantee shall obligate/spend no more than S_22,M,00
to carry out the Protect activities. The Grantee is onsible for any funds amended or obligated that exceed
tileAgreement amount, Funds unromIttod at the termination of the Grant period must be returned to lilt
Dewnment.
5, PERFORMANCE OF WORM
The Grantee will use the fundi granted by the Detainment for the purpose of carrying out the project
deecribal in Hider A,
6. INDEPENDENT CAPACITY
The Grantee and her/his agents and employees are acting in an independent capacity and not is an offi
anployees, or agents of the State, Grantee understands that sheAlc will accrue no retirement IarmJits,
vacation, sick leave or any other benefits available to State employees.
T. PAYMENTS
Thu Department will make payment in advance to the Grantee in four installments; However, don Depart
mem reserves the right to withhold payment if required reports are not submitted accordin0 to schedule.

10. OWNERSHIP

The Grantee will make the above: records available to the Departments Grant Manager, the Departments
Auditors, the State Auditor, and Fee l Auditor at the Grantee's headquarters at any reasonable time during
the duration Of the Project and for the three-year period alter the Project's conPletion.
13. SUBCONTRACTING

19. CHANGES IN THE PROJECT PLAN

15. TRAVEL
Prior autbeiiation is required for any, travel felt of state or aur -of country. Reimbursement for mileage

17. STANDA RDS OF CARE
Grantx agencies providing preventive services to clients shall use the Bureau of Health's Standards of
Gare as minimal standards, when such standards exist.
18. INDEMNIFICATION
The Grantee will defend and hold harmless the State and its officers and employees against any damage(•
or losses suf lured by any peace. firm, or corporation in connection with the performance of the work tender
this Agreement.
19. TERMINATION
Either party hereto may, by giving at least thirty days written notice, terminate this Agreement. Upon
termination or expiration Of this Agreement, as provided herein, the Grantee shall mour no further costs.
The Grantee agrees to liquidate all approved costs incurred during the period of the Abreemenl wit in six
tenths of the termination or expiration date, Any such termination shall not affmtany obligations or liabili
ties incurred by either Party prior to well termination.

ferrals from any community source and will assess for on-going health

The public health nurses will realistically help family and infant achieve

<) The public health nurses will continue to conduct two immunization clinics
monthly for children ft= the age of two months. Bangor residents as

against tetanus, polio, diphtheria, pertussis, measles, mumps and rubella.
d) the public health nurses will do school health nursing to six parochial
schools. she reviews and recommends immunization records upon entrance,
vision and hearing screening, staleosia screening and is available for
both teachers and students for consultation. Appropriate referrals are
made after screening procedures.
e) Rame visits for general health promotion are made to families requesting
Public health nurse Or to those contacted through referrals. Health
education material IS made available to families and this includes in-
formation for expectant parents, growth and development in children,
responsibilities of parenting, accident prevention, nutritional brochures,
baby care, pamphlets on Infant fending, discipline, toilet training, dental
health, specific topics such as BIDS. birth defects, diabetes, etc. Rome
assessments made by public health nurses include environmental Inspection$,
evidence of abuse and/or neglect, the intellect and emotional status of
family A$ well as homes providing basic needs for healthy children.
f) The public health nurses will keep updated on current issues as cell a
reviewing traditional nursing procedures by attending workshops, keeping
abreast on current literature. The public health nurses are often invited
as guest speakers to organizations to help Promote public awareness
of the
responsibility of citizens in maintaining public health at a optimum level.
Case conferences and team meetings regarding individual families include the
input from the public health nurses.

The responsibility of o Bangor Public Health Nurse under the Department of
Health and welfare 1s to first and foremost promote health and prevent

methods and technique utilized by the public health nurse to affect a change
in health responsibility are best promoted by 1) swing situation
accurately, 2) knowing up-to-date knowledge of cause versus effect,
3)giving one-to-onm contact with individual or mother/child duo 4)
Promoting mutual trust and respect for both public health nurse alul client
being served, 5) addressing the current or most glaring needs of client
6) having skill in interjecting health education into clients' daily life.
