HomeMy WebLinkAbout2018-03-26 18-152 RESOLVE 03/26/201818-152 03/26/2018 18-152 Health & Community Services N/A Accepting and Appropriating an additional $45,786.00 to continue the Bridging Nursing Services Grant from the Maine Department of Health and Human Services, Public Health Nursing Department through December 31, 2018. This Resolve will accept and appropriate an additional $45,786.00 in grant funds from the Maine Department of Health Human Services, Public Health Nursing Department to support specialized maternal & child health nursing for families in Bangor who are affected by substance use disorder during the prenatal, post-partum, and newborn stages of life. A City of Bangor Public Health Nurse has been specifically trained to provide care using a strengths-based, trauma informed, wrap-around model that assists the family with accessing available community resources. Referrals to this program are received from primary care providers, the EMMC NICU Champ Program, and the DHHS Office of Child & Family Services. If approved, city staff would also prepare and implement Memorandum of Understanding (MOU’s) with other agencies to coordinate referrals and other aspects of service delivery. An additional $45,786 in grant funds will extend the term of the grant from April 1, 2018 to December 31, 2018. The term of the Bridging Nursing Services grant is amended from August 15, 2016 until December 31, 2018 for the total sum of $144,989.00. 03/19/2018 Government Operations Committee 50 Recommend for passage First Reading 03/26/201818-152 03/26/2018 18-152 Supica Accepting and Appropriating an additional $45,786.00 to continue the Bridging Nursing Services Grant from the Maine Department of Health and Human Services, Public Health Nursing Department through December 31, 2018. Cathy Conlow, City Manager, is hereby authorized to execute a contract with the Maine Department of Health and Human Services, Public Health Nursing Department, in the amount of $45,786.00 to extend the Bridging Nursing Services Grant from April 1, 2018 through December 31, 2018.
