THAT theCity Manager be and is. hereby authorized to enter into an agree-
ment with the Department of Health add Welfare of the State of Maine relative
to conducting a survey of community resources for the care of the chronically
ill add the aged. A copy of said agreement is attached hereto.

PRCPOSED CONTRACT BETWEEB CITY OF BANGOR
AND STATE OF MAINE D ARTSCCT OF HEADTH AND WELFARE
The City of Bangor agrees to direct and conduct a professional
study including, but not necessarily limited too the following
elements:
(1) To conduct a study of the ways in whinh the facilftiess
resources and services available in Bangor can beat be
mobilized to care for the chronically in and agel
population of this area.
(2) To determine unmet medical and social needs in the
community in the general area of the chronic diseases
and the aged.
(3) To determine the necessary working relationships in which the
General Hospitala; the Bangor City Hospital; Private Nursing
and Convalescent Haves; and Private and Publics Official and
voluntary Health and Welfare Agencies; should participate.
()i) To determine the resources which need to be developed to
provide adequate and econard.cal chrncic diaeaae services and
services for the aged.
(5) To assign priorities to three needs to be mets and set forth
the sequence of development along with proposed financing
by the City ar other agencies.
(6) To set forth the training and recruiting that may be required
for institution of initial phases of the final plans and to
indicate what appear to be reasonable time lialta.
(7) To prepare for the State Departaent a Health and Welfare a
final report on the study for use by the Department in its
own Clearing or prograass or to assist other cowmsunities with
similar problems. This report is to set forth the metbod-
ology used, the factual data gathered and the plane and con-
clusions that are reached.
(b) The City agrees without charge to the project to select
a qualified person to conduct this study, end provide
necessary supervisions and direction. The City further
agrees to provide without charge to the project general
overhead services not requiring specific a pwatures.
(9) The Department epects that the methodology of the study
will permit some degree of participation by lay people,
and representatives of community agencies, and that the
finding of the study will he given a reasonable degree
of public dissemination.
(10) The ➢apartment agrees to reimburse the City of Bangor
for 75% of all expenditures properly cbargeable to the
project, up to a mand,mam amount of $h,CCC.0O3 Interim,
partial payments may be made to the City by the Depart -
mast, and will be based on progress reports, and estimated
expenditures. The local share of the financing of this
project may be in the form of direct cash expenditures,
or the dollar value of contributed services, or direct
cash grants from local agencies.
(11) Final payment to the City will be made when the project
is completed, ond an acoeptahle report based on the
points set forth above is transmitted to the Department.
(12) This entire agreement is made contingent upon the avail-
ability of a wrisetant satisfactory to the City of Bangor
both as to qualifications and coat. This agreement is
contingent upon final approval by the Bangor city Council
of an appropriation to ower the necessary local share.
