Commentary:
The City recently accepted proposals regarding an Employee Assistance
Program. Interviews were held with the two top responders and
volved representatives of the union and non-union ork force,
City management staff and a member of the City Council. The unan-
imous decision of the interview panel was

Care was most capable of performing the v required.
The Employee Assistance Program provides assistance to employees
who have personal problems that may be affecting their job performance
Types of problems involved may be physical, mental o motional
illness, financial, marital or family distress, alcoholism, drug
abuse, legal issues or. concerns. Confidential assessment and referral
services will be provided to employees free of charge by trained
Counselors. -
The adoption of this service/program has been a high priority of -
both City employees and Management staff.

This Agreement shall govern the relationship between Affiliated
Healthcare Systems (ARS) and The City of Bangor (Bangor) for the
provision of Employee Assistance Program (EAP) services.
1) AMS will provide Bangor with BAP services in accordance with
the "AHS' Proposal to Provide Comprehensive Employee
Assistance Program Services to The City of Bangor employees
and their families - February, 199010, including but not
limited to:
a. In conjunction with and on behalf of Bangor,
operating policies and procedures that meet the regula-
tory requirements for EAPs imposed by the State of
Maine.
b. Initial and ongoing training and orientation to
Bangor employees and managers according to formats
and schedules agreed to by Bangor and AHS.
C. One year (12 brochures -posters) of love -Life
wellness education material for distribution to Bangor
employees according to a schedule established by Bangor.
d. EAP oriented posters for display.
e. An ongoing promotional campaign.
I. An annual evaluation of the EAP for Bangor
representatives, as well as copies of AHS -EAP Client
Satisfaction Survey as they are received from Bangor
employees and dependents.
g. The entire AHS' BAP network of coordinator
professionals throughout Maine to providecomprehensive
assessment and evaluation services to Bangor employees
and dependents an an as -needed basis.
h. Indemnification of Bangor from any liability, lose or
expenses arising out of any act or omission of AHS
regarding the provision of services.
2. Asac ondition of offering BAP services thru ABS, Bangor
will:
a. Provide appropriate ongoing support in program
development, including supervisory training and general
employee orientation, support in program evaluation, and
availability of staff for assistance in resolving any
problems specific to protocols, policies and procedures
pertinent to the EAP program.
b. Indemnify ABS from any liability, loss or expenses
arising out of any act or omission of Bangor regarding
the provision of services.
3. For the above services, Bangor will reimburse AHS as
follows:
a. For Develenment and Trainina services outlined in "The
Proposal" a one time fee of $4,000 (based on 500
employees) plus travel expenses, including printing and
distribution of all training manuals and ABS general EAP
brochure.
b. For Implementation and coordinator services outlined in
"The Proposal" an
annual fee equal to $5,250, or $10.50
Par employee times the number of employees on payroll
as of July 1st of each contract year, whichever is
greater; plus $65.00 per service hour for individual
contacts with Bangor employees and family members by ABS'
EAP Coordinators.
C. Bangor will be invoiced:
1)
For one-third
of
the Development
and Training
Services Fee
on
the anniversary
date of the
contract;
11)
For the annual
fee
on the anniversary
date of
the contract;
iii)
monthly for all
coordinator services
according to
the above terms:
iv) except, that the total annual fee shall not exceed
$10,129 or $20.26/employee, which -ever is greater,
for each year of the contract.
Invoices are due and payable upon receipt.
This Agreement is valid and in effect for three years from July 1,
1990 to June 30, 1993 and thereafter shall automatically be renewed
from year to year, unless terminated by written notice of either
party no later than March 1, 1993. Any of the matters of the
Agreement herein contained may be altered from time to time by
written consent of AMS and Bangor without in any way affecting the
remainder of the Agreement.
