THAT the City Manager, on behalf of the City of Bangor, Is hereby
authorized ani directed to execute a Grant Agreement with the Maine
Department of Human Services, a copy of which is on file in the Office of
the City Clerk, aM to take all other necessary action, including
execution of documents end contracts, far the purposes of providing services
to clients waved by the Immunization Program.

BANGOR CITY BOUNIZA'FION PROJECT
1990
The Bangor City Dnmunization Project will begin its 11th year of
operation Jantery 1, 1990. The Project staff consists of a full-time public
Health Educator. The director of the Bangor Oepartmenr of Health and
Welfare will serve as the Project Supervisor and Grant Manager.
This project focuses its attention primarily on Penobscot and
Pi scor
quis Counties and secondarily on Aroostook, Washington, henceak,
Franklin, Kennebec, Knox, Somerset, and Waldo Counties.
Project efforts will be communicated and evaluated through monthly
reports and Periodic staff meetings. A weekly itinerary is also completed
and reviewed.
The activities of the Project for 1990 include:
1) Collection, organization, and use of annual immunization assessment data
from schools, post secondary schools, hospitals, and Bay Care facilities.
Z) Epidemiologic follow-up of reported v t ne-preventable diseases and.
appropriate outbreak control measures aswell as follow-up of MSAEPI
(Monitoring System of Adverse Events Following Immunization) reports, in
concert with the Sure Immunization Program.
3) Revalidation of annual assessment data by record review.
4) Completion of requested data according to predetermined time periods
(such as a"2 Year Old Study or Clinic Reviews).
S) Educational efforts directed toward target groups regarding vaccines and
vac c -preventable diseases.
6) Acting as technical and/or program resource to the medical community or
general public following State immunization recmmeendxations.
7) Liason with Canadian officials regarding vaccine -preventable disease
issues.
8) Active disease surveillance of vaccine -preventable diseases.
9) Promotion of age-appropriate immunizationa nitiatives and encourage the
use of State of Maine Standard Record Card,
10) Network with other groups who serve 'high-risk children to ensure
age-appropriate immunization.
11) Control of program resources, such as vaccine, to reduce wastage.
12) Development Of ro

immunization of target populations, such as preschool and adult.

The Bangor Department of Health and Welfare agrees that the public
Health Educator will also provide:
1) A weekly itinerary from to the .State Immunization Program
2) A monthly activity report to the State Immunization Program.
