These services will consist of: 1. Provision of an administrative and clinical base for the two Disease Intervention Specialists (DIS) representing the Maine STD Control Program in the northern section, including Counties Penobscot, Aroostook, piscataquis, Hancock, Somerset, Waldo and Washinton. The epidemiologic services provided by the DIS will be bath self -initiated and assigned by the STD/HIV program Office in Augusta and will be in accordance with the DIS JOB DESCRIPTION (ATTACWMIN/ W1). 2. Physical examination, laboratory testing, diagnosis, treatment, prophylactic treatment for, and epidemilogic services for persons presenting to The Clinic as volunteers with, or contacts to, known cases of infectious sexually transmitted disease ($PD), regardless of ability to pay. These services will be in compliance with COC documents a) alit Assurance Guidelines for SID Clinics- 1966 (previously attached] a 1 1 reatwar eLmesprevrouTttached], as directed (waxen and selected males) will be provided. In addition, all persons receiving services shall be advised of their personal risk of MV infection and will be encouraged to accept confidential HN counseling and testing services as part of their STD visit. All services shall be provided to the client at the service fee of $20, or a capped fee which is acceptable to STD Control. 3. Anonymous and Confidential HN Counseling and testing services for persons volunteering these services; services provided will be in accordance with current guidelines as found in the Bureau of Health Decumbent HIV Counseling g Testing Cuidelines For Division Of Disease Control 1vipiayees or Contractees. 4. Clinic and epidemiologic activity will be documented by weekly submission of both morb'dit re ports on diagnosed and treated STD patients and other into rninvestt etive a erwork (ds requested by the St ntro rogram or ce on and the 21.2]eppoort showing epidemiologic activity. In consideration of the sum of $6_1_472, Danger City Health E ''Welfare shall provide the above services. This amount, in addition to patient fees and ocher clinic fees, shall provide the resources from which the SID Clinic will operate for the year. Clinic supply expenses shall be the primary 91-89 --Rider A (cont) -- [Danger STD/HIV Clinical 0 Epidemiologic Services] responsibility of Bangor City Health 0 Welfare and shall be drawn from Patient fees. Assistance to the clinical operation, shall be provided from the Wine SID Control Program, as follows: Medications to treat gonorrhea, syphilis and chlamydia patients/contacts shall be Provided to the clinic by the Wine STD Control Program. Chlamydia test kits for all examined women and all eligible male patients seen in clinic [to be included as part of the routine clinic service at no extra charge to the patient]. HIV Antibody Test kits for all SID clients confidentially served at SM visit [to be included as part of the routine clinic service at no extra' charge to the patient]. The following structural chain -of -command establishes a collaborative relationship between the Bangor Health 0 Welfare and the Wine M Control Program. As funding source, the Whine M Control Progran will be responsible for 1) stating program overview 2) setting forth program goals and objectives and supportive activities, and 3) collection of data designed to measure those declared activities. Bangor Health 0 Welfare will bear responsibility for the management and operation of the clinic including day-to-day management through supervisory authority over the disease intervention specialists and part time staff, on matters relating to extension of all services emanating from the clinic. Medical aspects of services extended, the effective parameters of which are described in stipulation R1 above, are further to be overseen by the physician serving as the Medical Advisor in accordance with the chart review mandates of the Wine Board of Medicine, [att. 03]. Achievement and documentation of epidemiologic indices relating to M Program goals and objectives will he the responsibility of DIS assigned to the clinic. Oversight and documentation of clinic income from patient fees and other services shall be the combined responsibility of the DIS and the administrator for Bangor health 6 Welfare. Generic DIS job descriptions, as included on the STD DIS Sob Descriptions apply to clinic DIS, as members of the Wine M Control program. Patient Pees Patient fees shall be requested of persons seeking services, and every attempt should be made by clinic staff to maximize fee intake. The amount of the patient fee for STD clinical services visit shall not exceed an amount deemed to be reasonable and affordable by the City of Bangor, M Control Program staff and M Control Program Manager. The present fee for clinical services is $20. It is understood by all parties concerned that no Person lacking the ability to pay shall be refused services deemed reasonable to that client's presenting complaint, It is also understood that, in order to allow the clinic to adequately support itself, that patients fee intake must be maximized in order to provide funds for purchase of clinical and printing supplies, on-site lab services, rent and other administrative costs of the clinic operation.. The present patient fee for anonymous HIV counseling and testing is $15, for which amount the clinic will provide pre-test and post-test counseling and a serologic test for HIV antibody which shall be purchased fans the Public Health laboratory.. Performs or assures similiar investigative inquiry of all known contacts to infectious cases of above-named infections. Pully documents performed investigations on reverse of Epidemiologic Report Form (29.36). Observes program guidelines as found in = CON ROL PROEM PROEOCOLS & PROCEDURES. Strives to meet commitments made in STO Control Program grant objectives relating to investigative activities. PRIMARY RESPONSIBILITY 20% Assures interview services for persons with sexually transmitted disease (STD), either Personally or through attending care providers. Promotes the services of Bangor BTD Clinic for the examination and treatment of Persons named as contacts. Observes program guidelines as found in STD CONIROL PRYSR M PRDTOCOES & PROCEDURES. Strives to meet epidemiologic indices of the M Control Program as Itemized in the STD Control Program Grant objectives. PRIMARY RESPONSIBILITY 1258 Provides HIV counseling and testing services as a state -certified counselor and phlebotomist for at least two hours per day at the SID Clinic on behalf of Banger City Health Department and the Maine AIRS prevention Program. Provides records of counseling and testing activities to the Office on AIDS. Provides self -initiated counseling and testing (Field Services) for at -risk persons encountered when performing field investigations and, after conferring with STD Program Manager, those encountered in field screening initiatives. All HTV services will be in accordance with ID.V CGN6ELING & TESTING GUIDELINES. 10% Provides confidential contact notification services as requested by individuals with confirmed HIV Infection. Such requests will originate as 1) a request from a counseled patient, 2) a request from the Office on AIM on behalf of a person counseled elsewhere, 3) an interstate request. Provides field counseling and testing services to contacts to HIV infected persons and completes disposition for those activities, in accordance with DMV COUNSELING & RESPIMG GITIDE.tl@S.
