application of the Combined Health Appeal for inclusion in the
1992 City of Bangor Charitable Campaign, I request the opportunity
to make an appeal to the Bangor City Council.
I appreciate your assistance in the application procaes and
also understand your responsibility in regard to maintaining
the provisions of the policy.

MEMBER AGENCY ACTIVITIES IN THE GREATER BANGOR AREA AND THE
SURROUNDING COMMUNITIES OF PENOBSCOT COUNTY
National Multiple Sclerosis Society: Has an established Bangor
Volunteer Council (William Devoe, Provident); active Support
Group meets
monthlyat the Bangor House; 200 clients served
In Penobscot County; active in education programming For clients,
families and health professionals: "Living With ME", a 10
session educational program hoe recently been canpleted at the
Bangor Hou (45 Participanta)l and in October1991 a conference
for !'The Family With ME" was held in Bangor with JG participants.
American Cancer Society: Has alocal office at 99 Main Street,
Brewer with Chris Stark as Area director (989-6550); hes an
established Local Volunteer Unit (Rhonda Shirland, President),
seta regularly at St Joseph Hcspital; Support Groups include
Beach To Recovery (Breast Cancer Patients),Nu-Voice Club
(Laryngectomy patients) and Ostomy Assoc (colon and Rectum
Cancer Patients; active public education programming for youth
and adults, to date 9,813 adults and 7357 youth have had educe=
tonal pre entatins; 678 Penobscot County cancer patients have
received patint s this past year; SS children with cancer
and their siblings attended Camp Rainbow in 1391, this is a one
eek camping experience (no cost to participants) at Camp Jordan.

a from the Greater Bangor Area because of the involvement of
the EMMC Pediatric Cancer Clinic and the local YMCA.
Pine Tree Society for Handicapped Children and Adults: All local
volunteer and client programs are developed, promoted and admini-
stered be Barbera Milllcs of Brewer (1366-9782) and Michael Noyes
of Levant (889-6697), they are paid personnel who keep all of the
programs available to clients in the area; Fine Tree Camp is
available for clients from the area
and has a wide range of
participants; all programa of thePine Tree Siciety are avail-
able throughout the state and they keep records an a statewide
basis only.
Cystic Fibrosis, Northern New England Chapter: A Cystic Fibrosis
Clinic at the Webber Bldg in Bangor is available to clients with

Alzheimer's Disease and Related Disorders Association: Has a
Support Group that meets monthly (Bernice Damon, Facilitator -

support group that meets monthly at EMMC; completed a patient

Kids with Diabetes at Hersey Bsptreat on Penobscot Bay, For the
whole familyi also provides Camp Grand for kids and grandparents
to train additional family caretakers; the Bangor area is
targeted in November and March for educational programa using
the Risk Quiz - this peat March senior citizen, xshe targeted in
the a Bangor High School Key Club joined other area Key

tations (close to BODD brochures were distributed in the Greater

clients from Bangor receiving a

clients with Muscular Dystrophy or naQrnmuscular disorders
associated with MO; frequent publiceducation programs

time of the telethon with volunteers from WABI-TV, Fleet Bank,
Bangor Elks Club 3244, ERA Dawson -Bradford and client families;
MOA also has volunteers among the ranMa of the Bangor Police
Department and the Penobscot County Sheriffs Department -e well
s the employees of Ames and Service Merchandise. All services
are coordinated with the local unit.
Arthritis Foundation: has the formation of a support group i
progress; has just completed a public Fair in (3) communities
n Hancock County and is presently working with Bheumetologlate
n Bangor to present a Public Forum In Bangor in the Fell.
They have also been involved in public education programs in
area schools this past winter.

has presented a Women and Heart Disease Conference In Banger this

recently completed a Heart at Work Program For all of the employees

American Lung Association: The following programs have been presented
during this past year in the Bangor Area or
are presently in the

Cessation, Freedon From Smoking, Smoke Free Maine - Story Book
Program with W1 -B2 TV, Future Worker Health Lung Safety at EMVTI,
Air Quality for School Portehle Claeareaps, Lungs at Work - High
Risk Industrial Programs, Air Quality Alerts - Ozone and Air
Quality with television meteorologists and the Bangor Doily Newe,end
Camp For Kids coordinated with Eastern Maine Medical Center.
They have also presented TB Educational Programs at various sites
and are In the planning stages For the presentation ofa Living
With Lung Disease- Conference.
Lupus Group of Maine: has a trained counselor

Fine Tree Epilepsy Association: Has an establised Support Group
that meets once a

School AlertPrograms for teachers antl other school personnel, and
has an active Training and Placement Service for Epilepsy Clients
in the Greater Bangor Area.
Sudden Infant Death Syndrome Alliance: Mrs. Thomas (Gail) Brown
s the volunteer contect person for the Bangor Area. Programs
are n place for one an
counseling, entry into the Compassionate
Friends Support Group, and special counseling for families who
have multiple births and one of the Infants dies from BIOS. Also
e parts of the SIBS Chapter protocol e new BIBS family will get
a public health nwree visit to assist them - this i a state-wide
program and i active part of what is happening in the Bangor
Area. the Chapteralso offers a program wherby a 5305 counselor
will go to the worksite of a SIQB parent and diacuaa with follow
employees whet has happened and "how to help" concepts.

accompanied our application.

to the mnount of funis they raise during the campaign.
6.2 In the e t that the City payroll systmi mast be expanded or
codified to aecmrtodare the finds or federations herein, the City
ai ager or his designee is authorizes to ivposa a fee, payable in
advance, by tha participating find or fecieration to defray the cost of
the expansion, or mdificatiw caused by such fwd or felaration.
7.0 LYA21Scation Procaine
7.1 Applications for qualification containing all of the irdowatpon
arca donmentation repaired by tha C abinad Charitable Canpaign policy
must be submitted by Sepocabez 20, 1991. Beginning in 1992, the
application rust F received no later than May 1st. The City will
notify applying fulls or faleratiws of their status m later than .Ione
1st of each year.
7.2 Inthe event that an application is denied, the reason for ash
denial shall be rade in writing ani the applicant shall be advisel of
the right to appall to the Barcpr City Council.
7.3 Organizations providing direct services to the citizens of Bangor
and not nearing policy section 4.111 but nesting all other xequirenents
of the policy, can apply for special exmption to coact a ca paign,
as long as such organizations have not done so in Ura past three years.
