Councilmen STATE OF MAINE 92-106 Immunization Program Program: Barger Health S Welfare Dept BUREAU OF EA HUMAN SERVICER BUREAU OF IIEALTH Grant Dates: I/1/92 - 12/31/92 Page 5 Of 38 PROJECT SUMMARY: Since 1979 the Bangor Health S Welfare Capt. Immunization Program has operated in conjunction with the State Dorcas of Health ImmunizatiOn Program In previdi g services to the residents of Northern and Eastern Mine. The project staff consists of one full-time Health Educator. the director of the Bangor Health and Welfare Department Serves as Project supervisor and Grant Manager. Project efforts are communicated through quarterly reports and attendance at periodic State Peiwization Program staff Meetings. 1992 Project activities shall include: 1) Collection, organization, and use of annual immunization a sment data from schools, post secondary schools, hospitals, and day cares facilities. 2) Epidemiologic investigation of vaccine preventable disease reports including implementation of appropriate outbreak central measures: follow up on all valine adverse assets. 3) On-site validation of humanization assessment data (i.e. school validation audit). 4) Tasks involving data collection and verification (i.e. clinic reviews, etc.). 5) Community saturation related to vaccines and vaccine preventable diseases. 6) Serveas technical and program resource to the medial cmmmenity, following state and federal immunization recommendations. 7) Serve as a liaison with Canadian health officials regarding vaccine preventable disease Issues. 8) Active disease surveillance of vaccine preventable diseases. 9) Promotion of age appropriate immunization Initiatives. 10) Network with special interest groups who serve "high risk" children to ensure age appropriate immunization. 11) Meritor program resources, including vaccines and vaccine shipping materials to reduce wastage. 12) Development of immunization interventions designed to increase age appropriate immunization of target populations. 13) Conduct health education projects targeting persons served by the Fenger Wealth and Welfare Department and Bangor City employees. 14) Participate in the provision of Immunization and SCD Clinic services at the Bangor Wealth and Welfare Department.
