'Last year's grant was divided into two -- Health Educator and Nursing. Bangor
Was awarded Both grants. These are one year grants with the possibility of
renewal for up to three years.
The total amount of this .year's grant is $68.970. The purpose of the program is
to coordinate and facilitate chlldhootl immunization foxva ccine-preventable
diseases and adult fl,, Hepatitis a and overseas travel.

The Bangor Dramatization Program proposes abroad based Nursing Services plan. The
Nursing Plan is comprised of three major men: I) coordination and delivery of immunization
services; 2)provision of technical assistance to the State Immunization Program; 3) development
and implementation ofclimad and nursing entrench activities.
The Nursing Plan goal for humanization services is to continue providing six public
immunization clinics per month (see schedule, Appendix A). Chains will be added during the
late summer as students return to school, and during the fall in luenm season. Persons needing
mmunization for overseas travel will continue to be seen by appointment. AD clinics will follow
the Standards for Pediatric Immunization Practices Clhdcs will be open to all persons requesting
services without regard to age, sex, race, religion, sexual orientation, residence, income, or
disability. All clinic sites will be handicapped accessible. Clients will be screened for vaccine
need and contraindications per Standards. After screening all needed vaccines will be given
sum bameausly. Clients will receive State of Maine immunization record cards to keep and chide
records will be kept on file at the Health Department. All state -supplied vaccine will be offered
Gee of charge, donations for administration will be accepted. No child will be refused
incrimination if the Parent is unable to make a donation. No physical exam will be required and
exams will not be available on site. Clients without identified primary care providers will he
referred to local services. Standing orders for vaccine will be obtained and reviewed on a yearly
basis. Emergency procedures will be developed and reviewed on a yearly basis. Emergency

equipment available at clinics will include: ammonia inhalants, Epinephrine (adult and pediatric)
Bristol jets, oxygen and airway supplies
The nursing goal for technical assurance is to provide profeesiomal nursing service to
meet the Agency's educational and direct cue needs. Bangor Program nursing staff will meet
individually with the State humanization Program Manager, Immunization Action Plan
Coordinator, and Some Immuni atlon Program Health Educators to assess area needs, determine
regional resources, and develop a prionfized needs list. This will be completed within the first
quarter after proposal implementation. _
Bangor Program nursing staff will meet with primary care providers in four
geographically identified areas with low immumbaaon levels to assess their needs and prionfies
for immuNation services. The assessment will be accomplished with fare to face and group
discussions, and written and phone surveys. This will be done by the end of the second quota
after proposal implementation.
Nursing staff will work with identified resources to develop strategies to meet identified
needs by the end of the third quarter alta proposal implementation. Staff will begin
implementing identified strategies by the end of the fourth quarter after proposal implementation.
Nursing staff will provide services to community organizations promoting immaniation
when requested. Staffwill meet with representatives of the w C program, during the first half

of the proposal year, and assist them in then evaluation of children's immwivtion needs and
provide sniff education as needed.
The Bangor Program nursing staff has set the following objectives to meet the goal of
achieving the outreacb and educational needs of the State Immunization nization Program:
I. Side lmmwization Program staffwill be knowledgeable as to the
Reaction, abilities and goals of the Nursing mdt within one quarter of flue
proposal implementation.
11. The Bangor Program nursing staff will meet with State Program office and
field Staff quarterly to determine needs for technical or supportive
services. The nursing staff will then provide technical assistance and
educational materials as needed to the State Program staff.
III. Nursing staff will develop a manual of nursing responsibilities and
procedures for the Stare Progmm staff for use during outbreak situations
by the end of the second quarter after proposal implementation.

The Nursing Plan calls for staffing with two registered nurses. One will work full time
and the other will work 30 hours perweek. In addifion a musing pool will b<mainmmed m
assistwithclimc staffing. Pool nursingtime will total anavetageoffivehoursperweek. AB
nurses will maintain State of Maine Professional Nurse licenses and CPR certification. Allwill
attend in-service transmit to maintain or upgrade their skills and certification.
Job Responsibilities will be divided as follows: The full time notes will be responsible for
development and implemenrafion of the outreach education and training program; clinic staffing,
recall, and public education; meetings attendance, report preparation, and in -services. The part-
time RN will be responsible for: Clime preparation, staffing, recall and record keeping ten
hours/week; outreach activities five hams/week; meetings in -services, and report preparation five
hourstweek.
The Bangor City Council has adopted policy designed to implement terms of me
Americans with Disabilities Act in all City departments and services. Appendix B contains a
copy of this document.
