Last year's grant (1/1/95 to 12/31/96) was amended to tun through June 30, 1996. This year's grant will run from July 1, 1996 to December 31, 1996. Funding for the six months is $40,450. This is level with last year's funding. For your information a copy of the goals and objectives for the grant is attached. GOALS, OBJECTIVES, AND SLD =IE5: 1. Danger STD Clinic will provide HIV Ab councelingdesting services ' A. Have available 9 least 30 time slots weekly for HIV testing appomhncuts 1. Appointments during daytime and evening hours 2. Maintain a minimum of 2 DHS certified counselors on staff 3. Stagger counselor time for optunum availability B. All STD Clinic attendees will be assessed for HN risk; high risk patients will be offered confidential and/or anonymous testing 1. Each patient is queried about Personal condom use and IDU bistmy 2. Several and STI? histories will be assessed at each visit C. Provide individual counseling, education and risk reduction information 10 each individual volunteering for testing 1. Only DHS candied counselors perform testing and contact notifimion 2. All clients wN be individually assessed for reasons for testing, risk factors and educated about risk reduction 3. Printed information specific to client concerns and risks will be provided to each person tested D. Provide rental resources for follow- up for individuals testing positive for H[V L Maintain referral lists for support, medical follow-up, memal health, dental health, chemical dependency and other actual services as appropriate 2. Make appropriate referrals as suggested by Maine BOH as recommended on HIV counsebnghesting form 02544 3. Iffier conduct notification assistance to all who test positive for HIV and _ perform notification services on request 2. Network with HN positive women to serve as speakers to help lead discussion groups as requested. B. Make long tam ongoing ippon services available to women who agree to take pan in this project. 1. STD/ HN clinic gaff will set up regular coatings to help monitor progress of individual client's goals, to help identify problems/needs as they now. C. Make referred; available as appropriate to help overcome barriers to women seeking to make long tens behavior charges to reduce their IBV risks. 1. Arrangements will be made to make child care, transportation, etcavailable to assist women in keeping their appointments according to their care plan. 2. Refund fins will be maintained and updated to referwomen for counseling as needed for substance abuse, self esteem, dogmatic violence, enc.
