Item/Subject: Bangor Healthy Families Program/Contract

After several years in the planning stage, Bangor Healthy Families
has funding for a one year pilot program designed to Serve a small
number of first time parents who reside on the west side of Bangor
and meet the criteria which identify them as at risk of child abuse.
Staffing for the program will be in the form of an additional 8 hre.
per week added to the part time position of an already employed
Public Health Burse. All expenses associated with these 8 bra. will
be paid for by the Healthy Families Program.

the City manager is authorized to enter in to a memorandum of
Agreement with the Bangor Healthy families Program through The Child Abuse
Council, Inc'. D/b/a Parent Child Alliance for the purposes of offering
intensive visits by a Public Health Nurse to individuals identified as
meeting the criteria of the program. The agreement will be administered
by the City of Bangor, Department of Health and Welfare for the period of
September 1, 1998 to August 31, 1999.

The Bangor Public Health Nurse is not an employee of
the Parent Child Alliance or the Child Abuse Council,
Inc. and is an employee of the City of Bangor for all
purposes, to include payroll, withholding, health and
retirement benefits, professional liability and
workers' compensation and unemployment.
This Agreement shall be subject to change after
discussion and due consideration by both parties with
their mutual consent. Any such change must be stated
in citing and signed by both parties to be
enforceable.
This Agreement will be governed and interpreted
cording to the laws of the State of Maine and the
United States of America.
Parent Gild Alliance will:
Serve as the lead agency, general contractor, and
Parent Child Alliance for the Bangor Healthy Families
Program (hereinafter referred to as "Program").
Provide coordination for all components of the
Program.
Represent the Child Abuse Council, Inc. in
programmatic discussions and meetings.

MEMORANDUM OF lUIRSINOWT
For Nurse Home Visitor Services
Provide financial management for the Program.
Prepare and submit reports to external funding
sources.
Provide Healthy Families Family Support Worker
Training to the Bangor Public Health Nurse.
Reimburse Bangor Public Health Nursing for 8 hours of
home visiting services per week for 48 weeks based at
$20.62 per hour and reimburse Bangor Public Health
Nursing for fringe at 23% of the hourly rate.
Adjust the reimbursement rate of the Bangor Public
Health Nurse on July 1, 1999 to reflect coat -of -
living increases.
Reimburse mileage and other expenses reasonably
incurred by the Bangor Public Health Nurse.
r Public Wealth Nursing Will
Provide and designate a Registered Nurse to deliver
home visiting services to first-time parents residing
n the West Side of Bangor who are determined by the
Bangor Healthy Families Program to be at -risk for
abusing or neglecting their child(ren). Home visiting
services may include, but are not limited to in-home
nursing assessments for the mother and her child(ren),
screening children for sensory and developmental
delays, teaching parents about normal child
development, accompanying parents to medical
appointments, and coordinating referrals to commodity
agencies as appropriate.
Supervise programmatic staff and student interns as
appropriate.
Participate in the Healthy Families Family Support
Worker Training.

9. Participate and represent Bangor Public Health Nursing
In all programmatic meetings.
5. Provide service data to the Parent Child Alliance.
6. Prepare and submit monthly expense reports for
reimbursement of eligible expenses (including travel
at 29 cents per mile) to the Parent Child Alliance
beginning October 1, 1998 and following on the first
of every month.
7. Prepare and submit quarterly invoices for
reimbursement of home visiting services to the Parent
Child Alliance beginning October 15, 1998 and on
January 15, 1996; April 15, 1999; July 15, 1999;
October 15, 1999.
