Performance measurement
A staff completed post-pressmation form will document the number of women receiving risk
reduction iMmmation and the number of those expressing a desire to implement at least one
behavior change to reduce risks.
Coupore redeemed for clinic services will serve as a measurement tool as well. Percent of
coupons redeemed each quarter will be noted.

40% of individuals presenting for HIV counseling and testing will report attempts to modify risks
for STD and HIV.
Strate¢ies
I. Provide individual risk assessment for each client requuting counseling/testing services
and provide risk reduction education specific to client's behaviors
2. Negotiate with clients regarding risk reduction behavior changes to be implemented
between pre and post-test visits. Document client plans on counselor intake form.
3. Review risk reduction plan at post -ten; identify and document adherence to risk reduction
plan
4. Assist client to identify additional needs for risk reduction behavior implementation and
document on counselor form.

Counselor intake forms wit serve as the measurement tool. Information from pre- and post-
sessionswill bereviewed for elements of risk behavior changes. Tlwpercentageofclientswho
return for results and report adherence to at least one risk reduction step will be noted as
successful behavior change.

Continue participation ba the BN seroprevalence study conducted by CDC.
All STD Clinic attendees having a serological test for syphilis will be included N double
blinded seroprevalence testing for EBV Ab.
All clinic attendees presenting at initial visit will be asked to complete questionnaire
assessing HIV risk.
Provide community education on SMMW.
Present accurate, up to date information about STD/IBV to community schools, groups,
etc.
Maintain availability as training site for medical personnel from Family Practice program,
