Item/Subject: Immunization Program Grant Renewal -- Field
Responsible Department: Health and Welfare
This is the eighteenth renewal for the Immunization Program
Grant. This year, the Field Services Grant includes an
increase in duties and the addition of a second Health Educator.
The total amount of this year's grant is $105,000 which represents
total funding for the program.
The purpose of the program is to coordinate and facilitate child-
hood immunization for vaccine -preventable diseases. For your
information, a copy of the description of grant activities has
been attached.

the City Manager is authorized to apply for and accept from
the State Of Saint, Department of Human Services, Bureau of Health a grant
in the amount of $105,000 for continuation of imanization services
(field) to be administered by the City of Bangor, Department of Health
and welfare for the period 1/1/98 to 12/31/98.
