Councilman MEDICAL RECORDS CLERK 113 DEFINITION This is specialized responsible work involving the collection, completion, filing transcribing, 5nammarizing and reporting of social and medical data on patients in the City Nursing Center. An employee of this class is responsible for wora g with the Medical Director, attending physicians, Director of Nursing, Account Clerk, Utilization Review Physicians, Social Serviee Workers, Administrator aki other appropriate staff members and representatives of state and federal agencies, in the management of patient records and reports. ILLUSTRATIVE EKAA3LE5 OF WORK Prepares identification and smrmary sheets including patient's homes, social security n'mber, marital status, age, sex, home address and religion; addresses and telephone numbers of referral agency and hospital, personal physician, c utist, next of kin or other responsible person, admitting diagnosis, final diagnosis, condition on discharge, disposition of patient and any other data required by the state or federal agencies. Records medical evaluations, including medical histories, physical examinations, diagnoses and estimations of restoration potential. Compiles and records information regarding hospital diagnosis from accessory discharge sheet, reports from hospital attending physicians, or transfer form. Records andcompiles physicians' orders, medications, treatments, dietary information, restorative and special medical procedures required for the safety and well-being of the Patient. Records physician's progress notes describing significant changes in the patient's caditionwritten at the time of each visit. Records and compiles nurses'.notes containing observations made by the nursing personnel. Records and ompiles medication and treatment records including all medications, treatments and special procedures preferred for thesafety and well-being of the patient. Records and compiles reports from laboratory, x-ray, dental services, social service and consultation reports a well as patient care referral reports. prepares abstracts of all records as required. REQUIRED KNOLVI.IDGES. SULLS AND ABILITIES Considerable knowledge of state and federal regulations pertaining to Medicare, Medicaid and other related record processes. Ability to transcribe dictation from dictating machine and to operate a typewriter. Considerable knowledge of medical and nursing terminology as applied to nursing home operations. Ability to keep complex records, to assemble. and organize data and to prepare reports frown such records. Knowledge of office practices and procedures. Ability to establish and maintain effective working relationships with other employees, patients, the public and representatives of governmental agencies. Graduation from a standard high school. Considerable experience in performing stenographic and clerical work. Page 2 Considerable experience in the keeping of medical records aither in a hospital, doctor's office or related setting. Certification or registration as a Medical Records Librarian or Accredited Records TecNucian or, eligibility fob enrollment in a course of studies leading to such certification or registration. PATIENT ACTIVITIES COORDINATOR 210 DEFINITION This is responsible professional work involving the planning, developments administration and coordination of recreational add occupational activitiee with therapeutic content for patients of the Bangor City Nursing and Health Center. A person in this capacity is responsible for working with the Administrators Director of Nursing, Medical Director and other staff members as well as outside public add private organisations, agencies and individuals in developing And conducting all forms of therapeutic value to the patients which may lead to o encourage restoration to self-care and resumption of normal activities. ILLUSTRATIVE E%AMPLFS cP W Plans and helps conduct activities for patients using to the greatest possible. extent community, social and recreational opportunities. Encourages patients to participate in such activities. Develops and provides suitable activities for patients unable to leave their rooms. Helps others on staff to encourage relatives and others to visit patients. orders and maintains supplies including audiovisual materials, crafts and art supplies and equipment adequate to satisfy the individual and group interests of the patients. Aids in the total occupational rehabilitation process of patients as determined by medical assessment sf patiants' capabilities. Prepared reports on above activities as required by Administrators Director of Nursing and applicable federal, state and local authorities. Works with Physical Therapist in the rehab process for patients. REQUIRED MCaMiDGES, SEALS AND ABILITIES Considerable experience and training in directing group activities. Ability to work aM communicate effectively with persons and groups and motivate individuals and patients. Ability to maintain a sympathetic attitude towards and harmonious relationships with patients. Considerable knowledge of hospital and community resources.. Ability to keep records and mske reports. Ability to follow oral and written directions in exact detail. DESIRABLE EDUCATION AND E%P Mad Graduate from a four year college a city with a degree in educations c physical educations recreation o related field. Experienced in the management and development of group activity programs, programs related to occupational or group recreational therapy or related areas in institutions or schools.
