ORDERED,
THAT the City Manager, on behalf of the City of Bangor,
is hereby authorized and directed to execute a Grant Agreement with
tha Department of IWman Services, a copy of which is an file in the
office of the City Clerk, and to take all other necessary action,
including the ezeeution of documents ani contracts, for the
pspcee8 of providing Venereal Masses clinical Services to
Patients seen at the Bangor Health Department.
Statement Of Pact: 'Inc City Council authorized a grant agreement
with the State on Nwenber 14, 1984, Cowcil Order 85-8 for a
Seven-month contract. This grant agreement is for the remaining
£iVe months teuoinating on Nwanber 30, 1985. The reason for the
splitting of the contract was necessitated by the State's funding
cycle.

The Grantee and the Department agree se follows:
1. Perforwance of Bork
The Grantee will use the funds .greeted by the Department for the purpose
of carrying out the project described i the attached Project Plan.
2. Independent Capacity
The Grant e
and his agents and employees a acting in an independent
capacity and not as officers, employees, orspeaks _Of the State. Grantee
understands that he will accrue no retirement benefits, vacation, sick
leave or say other benefits available to State employees.
3. Indemnification
The Grantee will defend and hold harmless the State and its officers and
ampdoyess cS&f at aqy deamges Or losses suffered by ary person, firm or
corporation in connection with the performance of the work under tbis
agreement.
4. payments
The Department will make payments to the Grentee promptly in accordance
with the schedule of-Paymenta specified in the Project Plan when the
Grantee submits written requests for payments and satisfactory progress
reports. -
5. .Department's Grant Manager
The Department will designate a person as "The Department's Gaunt Manager"
who will be the Department's representative during Lha period of this grant.
He will approve all payments to be made under this grant and has the
authority to stop the project or redirect the work if necessary to insure
satisfactory results.
6. Reports
Me Grantee will make written reports to the Department concerning progress
of the work on the project at least Once a mouth. e content and format
of reports shall be as required by theDepartment's Grant Manager.

Uponcompletion of the Project the Grantee will submit a written report
td the Department's Grant Manager describing the snubber of cmduct and
results of the Project.The report shall contain a description of all
activities under the project, an accounting of expenditure of Spent funds,
soul such statistics and/or other material as the Department's Grant Mansger
shall require. _
7. Canapes
All reports, systems, Processes, or other products of the Project are
the property of the Department and upon request shall be turned Over to
the Department.
B. Access to Records
The.Grantee will maintain complete records Of work performed and expenditure
of Brent Puede. The Grantee will make these records available to the
Department's Grant Mana�ager, the Department's auditors, the State Auditor,
and Federal auditors at the Grantee's headquarters at was, reasonable time
during the duration of the project and for a three year period after the
project's completion.
9. Subcontracting
The Grantee will not issue any, subcontracts for work required by the
project unless approved in the grant plan m otherwise approved in writing
Or the Department's Grant Manager.
10. Changes in the Project It
Cham es in the scope, method or finances of the Project which require a
significant departure from the approved project plan must be approved in
advance and in writing by the Department's Grant Manager. the Department's
Grant Manager may, order changes in the project plan if he deems that such
changes are required for successful completion of the project. If a change
order issued by the Department's Grant Manager requires a cbange in the
total funds required for completion of the project the Department will
make an equitable adjustment of funds.

other health care providers for the purpose of obtaining sexually
transmitted disease reports and overall cooperation in the
Department's sexually transmitted disease control activities.

Grantee agencies with 15 or more employees must have a written plan for
affirmative action signed by the chief executive officer of the agency which
contains the following:

The City of Bangor Department of Health and Welfare is
a full-time municipal human se agency providing
services to residents of the City of Bangor. Theme
services include general assistance, public health nursing.
