Councilman 90-86 Page 4 of 19 Alw�!W The grantee shall provide STD clinical and epidemiologic services to all persons presorting to Danger STD Clinic, currently located at 103 Texas Avenue, Bangor, Maine. 'These services will consist of: 1. Physical examination, laboratory testing, diagnosis, treatment, prophylactic treatment for, and epidemilogic services for persons presenting to Bangor STD Clinic as volunteers with, or contacts to, known cases of infectious sexually transmitted disease (STD), regardless of ability to pay. These services will be in compliance with CDC documents a) QUALITY ASSIMANCE GUIUELII FOR SN CLINICS - 1986 (previously attached) and b) 1989 SID lnbsn fN1' GUID]D,1 (att. p 1) as directed by the Maine Board of Medicine. Routine screening for syphilis,gonorrhea, trichmoniasis, bacterial vaginosis, and chlamydia .infection (women) will be provided. In addition, all persons receiving services shall be advised of their personal risk of HN infection and will be offered and provided confidential HIV counseling and testing services during their visit. All services shall be provided to the client at the service fee of $20. 2. For persons seeking HIV counseling and testing, counseling and testing for Human Immunodeficiency Virus (HIV) in accordance with current guidelines as found in the Maine Bureau of Health Document HN COIMSELIMG E TESTING QQmfifliS FOR DMSION OF DISfv1SE C(X?ML ]MPLOyE6S OR CIXiIRAC[EES. (art. 42) 3: Clinic and epidemiologic activity will be documented by weekly submission of both morbidity reports on diagnosed and treated STD patients, other investigative paperwork as requested by the STD Control Program, the quarterly 21.27 Report epidemiologic report, and the STD Clinic Quarterly Activity Report (included in att. (art. 83) In consideration of the sum of $56,175 Eangor Department of 'Health and Melfate shall provide the above services. Material assistance to the clinical operation shall be provided from the Maine STD Control Program, as follows: Medications to treat gonorrhea, syphilis and chlamydia patients/contacts shall be provided to the clinic by the gine STT Control Program. All HIS mileage costs which are incurred in epidemiologic field investigations shall be paid directly to DIS by the Maine M Control Program. The combined amount should rut exceed $150 per month. Chlamydia test kits for all examined women and all male patients seen as sexual contacts to chlamydia (probably beginning in second quarter(. HIV Antibody Test kits for all STO clients confidentially served at Sr0 visit. Phe following structural chain -of -command establishes a collaborative relationship between the Bangor City Health and Welfare and the Maine STD Control Program. As funding source, the Mine STD Control Program will be responsible for 1). statiryg program overview 2) setting forth program goals and objectives for epidemiologic F, clinical activities, and 3) collection of data designed to measure these activities. Bangor Impertinent of Health and Welfare - ill bear responsibility for the management and operation of the clinic and will strive to meet program goals. Medical aspects of services extended shall be be overseen by the physician serving as the Clinic Medical Advisor in accordance with the chart review mandates of the Maine Board of Medicine. Oversight and documentation of clinic income from patient fees and other services shall be the responsibility the department. Achievement and documentation of all epidemiologic and clinic service initiatives as found in the CDC funding grant and in SID COWI&OL PROIRNM pRDT➢COIS B PRIX:DTIIXPS - Jan. 1990 shall be the responsibility of DIS assigned to the clinic. Generic DIS job descriptions, as included on the SID HIS Jab Bescription (arts N ) apply to clinic DIS, in their capacity as members of the Maine SID Control program. patient Fees Patient fees shall be' requested of persons seeking services, and every attempt should be made by clinic staff to maximize fee intake. It is understood by all Parties concerned that no person lacking the ability to pay; shall be refused services deemed reasonable to that client's presenting complaint. It is also understood that, in order to allow the clinic to adequately support itself, that patients fee intake must be maximized. 9P-86 Page 6 of 19 RIDER A (coni) Patient Fees (con's) The present patient fees for anonymous HIV counseling and testing is $15, for which amount the clinic will provide pre-test and post-test counseling and a serologic test for HIV antibody. Test kits are available from the Public Health laboratory for $8.50. If the capped fee amount is raised to $20, Danger Health and Welfare is free to request this amount from clients. Contributions from HIV clients can also be accepted.
